Passports

Professional Document Solutions can hand deliver your passport to the U.S. Passport Office to help expedite your request. Certain guidelines must be followed:

1. Completed DS-11 form:

- DO NOT SIGN. This form must be executed in person at your local post office or clerk of court (Acceptance Agent). The following items must be brought to the AA and submitted for the AA to seal in the envelope. Please advise the AA that you are using Professional Document Solutions for expedited handling to the passport agency.

Proof of U.S. Citizenship

- Previous passport
- If you were born in the U.S., a certified copy of your birth certificate issued by the state, city, or county of your birth

Two Photographs

- Photographs must be taken with last six months, identical, 2x2 inches in color or black & white and must show a front view, full face, on a plain, light colored background (NOTE: Vending machines photos are not acceptable)

Proof of Identity

- A previous U.S. passport, a Certificate of Naturalization or Citizenship, a valid state issued driver’s license, government or military ID issued at least 6 months prior.

Fees

- New Passport $85.00 (age 16 and older);
- New Passport $70.00 (under age 16);
- Passport Renewal (if previous passport issued within last 15 years and is not damaged and you include in envelope) $55.00

Make check or money order payable to “U.S. Department of State”. If you require expedited service, you will need to pay an additional $60.00 expedite fee to have your passport expedited. These are the U.S. Passport Office fees and do not include our service fees.

Social Security Number

- Although a Social Security number is not required for issuance of a passport, Section 6039E of the Internal Revenue Code of 1986 requires that passport applicants provide this information. Passport Services provides this information to the Internal Revenue Service on a routine basis. Any applicant who fails to provide the information is subject to a $500 penalty enforced by the IRS. Questions may be directed to the nearest IRS office.

Name changes

- If your name changed, enclose a certified copy of the Court Order, Adoption Decree, Marriage Certificate or Divorce Decree specifying another name for you to use. If your name has changed by any other means, you must apply in person.

Proof of Departure

- This is especially important if you require expedited service. Two copies of your plane ticket showing departure date or two copies of your itinerary from the airline or travel agency showing departure date. Only one copy should be sealed inside the envelope, the other copy should be clipped to the outside of the envelope.

Letter of Authorization

- Include a letter authorizing Professional Document Solutions to process your passport request. Please provide two original letters, one should be sealed inside the envelope, the other clipped to the outside.

2. Please send the sealed envelope to us along with the original letter authorizing Professional Document Solutions Inc. to process and pickup your passport, a copy of your itinerary or plane tickets, our fee (please call for rates) and your preferred choice of return shipment (include air bill to avoid additional shipping charge).
